Behavior Plan

It is my experience that every student has the potential to reach goals and understand consequences. Sometimes, a little reminder or contract is needed to help guide the student in the right direction. This year we will use this template as a behavior modifier in the form of a contract. I will also send a blank copy home, so that you can familiarize yourself and your child with it together. The outline of the behavior plan is as follows:

All goals will be implemented by your child and guided by myself. As a team, we will decide on appropriate rewards and consequences. Upon the review date, we will either reward and re-evaluate until all goals have been met with consistency. If necessary, we will schedule a meeting between the family and myself to review and re-evaluate together. As a team we will implement a plan tailored to your child's learning needs.
